Services listed below with same day cash pay discount:

  • Glasses Exam  $100

  • Spherical Contact Lens Exam  $135

  • Non-spherical Contact Lens Exam  $150

  • Refraction Only  $75

  • Contact Lens Training (mandatory for new wearers)  $40

  • Optos (digital retinal images)  $25

  • Office Visit  $90

 

Notice:

 

As of January 1st, 2019 we are no longer able to bill VSP, Spectera or Davis for vision insurance. If you have one of these vision insurance plans, we will provide you with an itemized receipt if you are seen at our office so that you can use to self submit for reimbursement. You will then get reimbursed directly according to your plan provisions.

How to submit for reimbursement? Contact your insurance plan to check if your plan includes out of network benefits. Ask them how to submit a claim for reimbursement. Most insurance companies require a copy of the itemize receipt and completing a form specific for your vision insurance to request reimbursement.
