Please Note The Important Information Below :
Do you have VSP, Spectera, United Health Care (UHC), Davis Vision, FEP Blue, National Vision Administrators (NVA), Avesis, Superior Vision, Vision Benefits of America (VBA), University Health Alliance (UHA) for vision insurance? If you have one of these vision insurance plans or one that is NOT listed, we will provide you with an itemized receipt, then you may self submit for possible reimbursement. The reimbursement should be directly mailed to you according to your plan provisions.

How to submit for reimbursement? Contact your insurance plan to check if your plan includes out of network benefits. Ask them how to submit a claim for reimbursement. Most insurance companies require a copy of the itemize receipt and completing a form specific for your vision insurance to request reimbursement.
